Pavement Tarmac Sticking up

Right outside the children centre where the childrens centre meets the green is there is damage to the pavement around 2-3m long and tarmac sticking right up in part of this, It looks like someone could injure themself, its a very busy section as its on the school route, please investigate asap,

    This drainage cover needs urgent repair as a fall hazard. It's right outside my driveway. Noticed this morning.
