Raised tarmac causing injury

Outside the entrance to the Manor Farm club on the Portway leaving Bristol, there is a utility cover and tar mac around it that looks like it has been at some point replaced. Cyclist has injured themselves on a small section of tar mac that is raised approx. 3 inches approx. 18 inches put from the kerb while cycling. Went over handle bars, ended up approx. 25 metres down the road sustaining grazes to right arm, knee and shin. left hip and shoulder. Damage to two knuckles on left hand and loss of a fingernail which now requires surgery. Please investigate
