Defective paving slab causing injury

At approximately 2.15pm on the afternoon of 13 January this year. Ms Stephenson was walking through Victoria Square Gardens in Clifton from the direction of Clifton Village towards Lansdowne Place. Approximately 30 – 40 yards along the path she suddenly caught her foot on a defective paving slab (see photograph) causing her to trip and fall. She was wearing flat boots. As a result of her fall she suffered a significant injury. We would like to know the date of the last council inspection prior to Ms Stephenson's accident.
