Metal cover damagd and partially raised

Damaged metal cover on footpath in front of no. 16 Promfrett Gardens (bottom of Burfoote Gardens). The cover is bent in, no markings and the tarmac surrounding is failing - breaking away. Metal lid bent by 5cm/1". Needs repairing as hazard.
