Dangerous, cracked & uneven pavement caused by tree roots

It is with some urgency that I would ask you to take action regarding the dangerous condition of the pavement outside our property at 61 Filton Avenue . The tree roots have raised and cracked the surface significantly, making it very hazardous to pedestrians (especially the elderly & disabled). Over the last few months we have witnessed people tripping and stumbling and we feel it is only a matter of time before a serious accident occurs. With dark nights on the way and possibly snow & ice through the winter months, this could undoubtably lead to someone receiving a very bad injury.
While I am obviously not an expert in this field, I think the only permanent solution would be to remove the tree concerned and possibly replace it with a sapling.
Repairs have been attempted on previous occasions but the problem is now far worse.
