Ill fitting water gully cover. Cast Iron Rocking on two pins. Regular traffic using- I'm sure will fail

The last few weeks the gully cover is rocking on two pins. It is located next to pavement but is travelled over regularly due to location at the speed limit of 20 mph and by heavier vehicles. It will be dangerous once one of the two pins fail.
