Damaged pavement around manhole/manhole cover loose

Was walking up to melvin square when i tripped on this and fractured my elbow. This was around 8:30 at night and was in a blind spot to street lighting so it was completely black. Not only is the area around the manhole cracked enough its very easy to trip on, the lid itself is loose and only with a little pressure will move.
