Dear Sirs
I would like to report a recurring problem.
I first reported this as a problem February 2017, following which the Council eventually carried out the necessary repairs November 2017.
Unfortunately, the same utility cover and the frame within which it sits are again both loose. As a result, as before, every time vehicles drive over it, both the cover and frame rattle very loudly. This is a serious noise nuisance and is getting worse. I can only describe it as a very loud bang / crashing sound and it is now so loud that it is keeping us wake at night.
In addition, the tarmac / road surface around the edges of the cover is crumbing / breaking away. This may be why the utilities cover is rattling.
Any thorough inspection of the same should only conclude that it presents a serious health and safety issue - particularly to cyclists.

The problem is worsening.
This is a real noise nuisance, not to mention a H&S issue.
Please remedy urgently.

Please confirm what if anything has been done to remedy the issue flagged 24/03/20.
I received an update 24/03/20 confirming: "This issue is currently under investigation. We'll provide updates on the outcome as soon as we can.". However, I have not heard anything further and the problem is worsening.
I look forward to hearing from you without unreasonable delay.
