Damaged path and loose slabs

Sent to Bristol City Council less than a minute later. FixMyStreet ref: 2224299.
This has been damaged since i moved in a year ago. I am now reporting it as a elderly lady has recently tripped over a loose slab. The pathway is uneven, two large slabs are completely loose and raised. The dropped kerb is damaged and there are many variation to the height of the path.
