Very uneven raised pavement slabs on a steep bit of path

Sent to Bristol City Council less than a minute later. FixMyStreet ref: 2543357.
The issue has been present for some time someone told me. It's on the bend of the path with a cycle lane running alongside it. The slabs are raised in different directions and present a tripping hazard. My wife unfortunately tripped due to the uneven surfaced and fell front first down the path burning holes in both her gloves and coat when she landed and slid along the pavement. She also damaged her arms on impact
