Loose, moving pavement slab- major trip hazard

I was on a jog 2 days ago along Mina road and tripped and fell over this slab, which is just on the corner by roseberry avenue, on the side nearest to the m32. This is a usual route of mine so I think this is a new problem. The fall was so severe I ended up cracking one of my front teeth. As well as sticking up dangerously, the slab also moves around when standing on it.
