Substandard repair to mainhole cover

See the attached photo.
It looks like this was recently worked on, however...
- The tarmac lift around it is raised above the level of the road - The metal cover is sunken down below road level - AND the cover is at a 30° angle to the road. - The cover has started to sink more over the past two weeks and is rattling when ran over by vehicles.
Obviously an apprentice job. Needs to be investigated to whom actually competed the work because while the metal cover can be somewhat excused for sinking because the underlying road structure, the raised tarmac around it is blatant substandard repair.
