Broken around cover getting worse. And cover makes very loud niose noise when vehicles go over it

Its extremely loud. I can hear from my flat. It disturbs my sleep and many other people living closer than I do to the broken utility cover. The cracks around it are getting bigger and peaces of tarmac are coming off. The cover is sinking into the ground. This was reported and fixed! It had no. And is getting worse. And disturbing residents sleep
