Rocking and Rattling Utility Cover

The Utility cover on the road rocks and rattles every time a vehicle drives over it. This causes a nuisance to all of the houses in the area, especially at night or in the early morning. The Utility cover is either misshaped or was not installed level to the road surface. Please rectify this issue.
