Curb stone previously repaired is rocking up and down

This was repaired a few years back, but the same curb stone is now rocking up and down, with visible water/puddling underneath.

The issue started perhaps 2 months ago, but in the last week or so has just become really lose and is jumping up and down as cars go over it.
