Tree in poor health

Sent to Bristol City Council less than a minute later. FixMyStreet ref: 7197842.

This flowering cherry produces very few flowers, it is uneven. It is heavily infested with something that eats all the leaves when in leaf. It was hit by a car a number of years ago and there is damage to the trunk. I would like it be be removed and would be happy to pay for a replacement. If removal is not an option I would be happy to pay for it to be professionally pruned in the hope that it improves in health.
