Hole in pavement cracked

This crack/hole was caused by a skip when you replaced the roof of my house. I've reported this before but still nothing has done. I've tripped on this and twisted my ankle as a result. Im a disabled lady who struggles with balance. And this is at the bottom of my driveway. My house number is 59 ambleside ave
