Depression on the road that appeared again after fix

A large depression (deep enough to cause a significant impact to suspension) has reappeared on a previously patched section of the road after recent repair. Vehicles tend to go dangerously close to the opposing lane and the pedestrian island to avoid it. Requesting a thorough, permanent repair to prevent it from happening again.
