Tree roots pushing up pavement slabs.

The pavement slab has caused an accident to my grandad as he was walking home from Morrison's where he kicked the defective pavement slab and landed flat on his face (picture attached) the pictures also show blood on the floor as well. Considering there is quite a elderly community in Fishponds, this could have happened to anyone and needs to be addressed as soon as possible.
