This drain lid was not fixed on and flipped up when a 2 year old child stood on it

This drain lid was clearly not fixed a 2 year old child stood on it it flipped open and he fell down it!!! One leg was thankfully still on the grass and stopped him falling completely down. This is so dangerous and if I hadn’t grabbed him quickly he could have struggled and fallen down the drain
